ECOB Facility Request Form

Please fill out all sections of this form. Once this form is completed, the Operations Coordinator will review the request. This will typically happen on a Friday of the week the form is received. If approved, an email will be sent to the contact person with the facility usage guidelines and facility costs. A final communication that will ask for a signature and a deposit (if needed) for the facility's use will be shared. If the facility request is not approved, a reason will be shared with the contact person.

Please mark any part of the form with an "*" - you cannot submit it without these parts marked.

Filling out this form is the first step in the approval process. We will not book your reservation until all steps are met and approved.

Facility Reservation Process:

1) Fill out the Facility Request Form

2) Signature of contract and deposit made - (sent via email)

3) Approval or denial of the request - (sent via email)

4) Final payment made and arrangements for keys, AV, etc.

Please list below all of your group's needs. Each group is responsible for setting up and tearing down their allotted rooms. Each group is to leave the room better than the way they found it and in the same orientation they found it.

 

NOTE: Any AV need in the Community Room, Sanctuary, or Multi-Purpose Room will REQUIRE a trained AV specialist. This will be at the cost of the group renting the facility at the fees outlined in the Facility Agreement (approx. $25/hour)/
